Referrals

We accept referrals at our Therapy Services office at Slynedales directly from cancer patients and their carers, as well as from medical and nursing staff in local primary and secondary care settings and cancer networks.

Care planning

We work individually with each person who is referred to us, to clarify with them their needs and preferences and to agree the right mix of services for them. The agreed plan of support and care is reviewed with each individual as their needs change, if necessary in consultation with their medical carers. You will be met initially by one of the following members of the therapy team:
